Funding from the Department of Health enabled us to research the hopes, needs and wishes of people involved in person centred planning. They included people with learning disabilities, carers and staff at all levels.
The resulting pack, 'Shaping the Future Together', includes information to help managers and commissioners improve services based on detailed evidence collected from person centred plans.
