People need the right support in order to live life as they choose and be active citizens. Person centred planning and individual funding help people to have control over the way they are supported and who supports them. This is central to all our work.
Valuing Families in Kent is a project aiming to build the leadership skills of family carers and to give families a greater voice in the development of opportunities and delivery of support to young people and adults with learning disabilities.
Get a Plan in Cornwall was established to build capacity for person centred planning and circles of support. We do this by sign posting, offering training, mentoring and support for people in Cornwall around person centred planning and personalisation.
We Can Dream! details how person centred planning and self-directed support can be used by people with autism when making the transition from school or college to adult life.
This project focused on finding new approaches to help young people with a learning disability such as Down’s syndrome to lead an active life after school and college.
