Using Person Centred Planning
Person Centred Planning provides a way of helping a person plan all aspects of their life.
It is a method often used to aid adults with learning disabilities, but is now being used by schools to help students at their SEN transition reviews from year 9 onwards.
The purpose of Person Centred Planning is to:
- Encourage the person to think about their life now and in the future
- Allow them to make their own decisions and take control of their life
- Decide who they want to help them and how
- Help them build a Circle of Support
- Give them a voice
There are a number of key questions to consider when helping someone develop a plan, such as:
- What are the person's dreams?
- What are their strengths or gifts?
- What are their likes and dislikes?
- What is important to them?
- Who are the important people in their life?
- What support does the person think they need to achieve the future they want?
In We Can Dream! we explored how Person Centred Planning could be used to help young people with Autism make the transition from school or college to adult life.
We have also published a report that features four young peoples' experiences of using a plan, and have created a personal planning booklet to help them and others to do so.
